Health Care Home – a NSW Case Study

As the Federal government starts phase 1 of the Health Care Home (‘HCH’) programme this year, practices around the country prepare for the practice transformation that is required to adopt this model of care. With the help of the Western Sydney PHN, our practice has been changing and adopting Patient Centred Medical Home principles over the past 2 years. This presentation outlines the change concepts adopted, and will explore the quadruple aim of enhancing patient experience, improving population health, reducing costs and achieving joy in practice. We will also discuss the 10 building blocks and outline our practice’s approach and journey to whole practice transformation.
